In May 2013 the homepage carried a notice that the office on South MacDill Avenue had relocated, effective April 2013, to two new addresses — one in Tampa, one in Largo, Pinellas County — with new telephone numbers and an office email for appointments and records requests, and a line that the website “will be updated”. The addresses, numbers and email are not reproduced.
January 2014 · “Moving out of state”
By January 3, 2014 the whole site was one page: the physician was moving out of state and referring her patients to another holistic D.O. practising integrative medicine in Tampa, whose practice name, address and phone were given; patient records were available for a per-page fee by official request to the office email. “Copyright © 2014.” The referred physician’s details are not reproduced.
What came next
Public physician directories list a Dr. Jean Allen, D.O., with the same medical school and graduation year practising family medicine in California, which is consistent with the 2014 notice. This site has not verified that they are the same person and makes no claim about her current practice. The 2014 notice is the last trace of the practice on this domain.
